This claim form is for all persons or entities who, during 2012 or 2013, purchased drywall for end use and not for
resale in or from an entity located in Arizona, Arkansas, California, the District of Columbia, Florida, Illinois,
Iowa, Kansas, Maine, Massachusetts, Michigan, Minnesota, Mississippi, Missouri, Nebraska, Nevada, New
Hampshire, New Mexico, New York, North Carolina, North Dakota, Puerto Rico, Rhode Island, South Dakota,
Tennessee, Utah, Vermont, West Virginia, or Wisconsin.

If you know the cost of the drywall you purchased, please carefully complete the following chart by filling in the columns for each purchase made between 2012-2013. You may use multiple entries if necessary. Funds will be allocated to those who can establish, through the claims process, that they purchased gypsum wallboard between January 1, 2012 and December 31, 2013.
If you do not know the cost of the drywall you purchased (i.e. because you purchased drywall through a
contractor as part of a home renovation project and do not know the cost of the drywall installed in your home),
you may estimate your purchases by selecting the "add purchase information" completing the following chart. Do not include any purchases identified elsewhere in this claim form.
III. Proof of Purchase
To enhance the validity of your claim, please include a copy of your proof of purchase for each purchase identified in section II if you possess it. Please attach the proof of purchase(s) to this form or upload to your claim form on the settlement website.
Providing proof of purchase will increase the percentage of compensation you will receive. If you cannot provide proof of purchase, your allotment of the settlement will be reduced.
Your purchases will fall into two groups:
Option 1: Settlement Class Members who do not provide valid Proof of Purchase, and complete the Claim Form confirming under penalty of perjury (i) that they purchased one or more wallboard Products during the Settlement Class Period and (ii) the total sum of money spent by them on wallboard Products during the Settlement Class Period, shall receive payment in accordance with the Plan of Allocation, available in the 'Documents' section of this website; provided, however, that recovery under this Option 1 may not exceed $100; or
Option 2: Settlement Class Members who complete the Claim Form and provide valid Proof of Purchase showing their actual purchase(s) of wallboard Products during the Settlement Class Period shall receive payment in accordance with the Plan of Allocation, available in the 'Documents' section of this website.
To receive payment, a Claim Form MUST be completed, and either be submitted on this website or postmarked by January 9, 2019.
Class members are eligible for one claim submission per household. The actual amount a Class Member receives will depend on the amount of money available from the Settlement Fund described below and the number of claims made. If the total value of valid claims is less than the amount of money available to pay them, then Class Members submitting valid claims will receive the full amount of recovery they sought, and each award will be increased pro rata. If the total of valid claims exceeds the amount of money available to pay them, then each award will be reduced pro rata.

I also certify that this is the only claim I am submitting in connection with this case, and that I have not submitted a claim for the purchases identified in this form in the related Direct Purchaser Settlement.
